No one should go without the medications they need. The GAP Program is a medication assistance program designed to help close the gap between considering fertility treatment and actually getting it. The GAP Program is provided through fertility specialists at participating fertility clinics across Canada.
Your fertility specialist will know whether the GAP Program is right for you. The GAP Program allows participating clinics to provide assistance to qualifying clients for medications associated with fertility treatment.
To access the GAP Program, simply use the clinic locator to find a participating fertility clinic in your area (these clinics are highlighted with a special GAP logo) With a few simple steps, the GAP Program can help you get on your way to receiving the fertility help you need.
Access the
GAP Program in 5 easy steps:
Step 1: Use the "Fertility Clinic Locator" on this website to find a clinic of your choice.
Participating clinics will have the caption "Participating in GAP - a medication assistance program for fertility treatment" next to their clinic name. Submit your email on a clinic information page to receive a GAP/REACh certificate. An email will automatically be sent to you. Please follow the instructions on the email.
Step 2: Present your doctor with the GAP/REACh certificate. If your doctor determines this program is right for you, have them sign the certificate and return it to you.
Step 3: You must fax the GAP/REACh certificate, with your doctor's authorized signature, to 1-866-938-8673.
Step 4: You will receive a subsequent email which will provide you a unique GAP ID and confirmation of participation in the GAP Program (normally within 3 days). Your GAP ID will allow your fertility clinic to verify your eligibility and participation in the GAP Program.
Step 5: Print off the confirmation email with your unique GAP ID. Bring this to your next appointment at your selected participating fertility clinic as confirmation that you are enrolled in the GAP Program.
The GAP Program is continually striving to assist health care professionals in providing services to people attempting to have a child.
